Required for Every Tournament
Permission Slips & Carpools
All students must submit a permission slip to be allowed to attend a tournament. Pick up a slip from F201 or download and print. All permission slips must be submitted on paper to F201. Each permission slip is valid only for one tournament.
Carpool Drivers
Any parent wishing to carpool a student other than their own to/from a tournament must complete the carpool form. This year, all carpool drivers must complete a fingerprinting and approval process. Students will not be allowed to ride in carpools unless the driver is approved under this new process. Each student must be prepared to furnish their own transportation as carpools are not guarenteed.
